✅ Pre-employment drug test results
✅ Employment application — completed and signed
✅ Previous employer safety performance history — 3 years back per FMCSA requirements
✅ Annual review of driving record — completed each calendar year
✅ Drug and Alcohol Clearinghouse query results — pre-employment and annual
